New Client Engagement Agreement Federal law requires that a consent form be offered to every client. This electronic form satisfies that requirement. You are not required to sign, but by refusing, you are considerably limiting the services we can provide. Unless authorized by law or given express consent by you, we do not disclose, your tax return information to third parties for purposes other than the preparation and filing of your tax return. The individual(s) or entity representative undersigned below (Hereinafter CLIENT) expressly authorize McQueen Bookkeeping & Tax Service, its employees and/or assignees (Hereinafter COMPANY) to disclose necessary tax return information to TaxSlayer Pro or any other entity required for the purpose of filing my tax return electronically with the Internal Revenue Service. If you believe your tax return information has been disseminated unlawfully, you may contact the Treasury Inspector General for Tax Administration (TIGTA) by telephone at 1-800-366-4484. COMPANY will prepare CLIENT’S current year federal and state tax returns based upon the information provided by the CLIENT. Preparing CLIENT’S return does not include auditing and/or verification of information provided by CLIENT. This engagement does not include any pre-audit investigative time or examination of CLIENT’S books. If CLIENT’S return is audited, CLIENT is responsible for proving the information reported. The tax return preparation fee does not include charges for tax authority inquiries, requests for filings, audit representation or preparing materials for responding to correspondence from taxing authorities. Preparation fee does cover limited assistance and consultation during the engagement to prepare your tax returns.Assistance terminates upon delivery of completed returns and original documents to CLIENT. CLIENT agrees to keep supporting documents and copies of your tax returns in a secure place for at least seven years. COMPANY may at its sole discretion assess a fee for additional copies of filings and schedules. CLIENT agree to provide us all income and only legally deductible expenses. If additional information becomes available after we work on your return, CLIENT agrees to disclose the new information immediately so completed tax returns contain all relevant information. CLIENT affirms that all expenses or other deduction amounts are accurate and that you have the required supporting written records. COMPANY may ask to review CLIENT’S documentation if necessary and CLIENT agrees to cooperate with that request. CLIENT agrees to provide legally sufficient substantiation if audited by either the IRS or state tax authorities. COMPANY will provide guidance concerning what constitutes acceptable proof, if asked. CLIENT agrees to review the return carefully before signing to make sure the information is accurate. Fees must be paid before CLIENT’S tax return is delivered to you or filed. If CLIENT terminates this engagement before completion, CLIENT agrees to pay COMPANY for all work completed and any preparation time expended. CLIENT agrees to pay COMPANY an upfront nonrefundable fee for preparation of late returns. The nature of our work requires us to collect certain nonpublic personal information about you from various sources such as applications, worksheets, reporting statements, bank statements, credit and other sources. All information we obtain about you will be provided by you or obtained with your permission. Our firm has procedures and policies in place to protect your confidential information. We restrict access to your confidential information to those within our firm who need to know in order to provide you with services. We will not disclose your personal information to third parties without your express permission, except where required by law. We maintain physical, electronic, and procedural safeguards in compliance with federal regulations that protect your personal information from unauthorized access. CLIENT agrees that the electronic signature binds the same as if it was affixed with a handwritten signature.
